Prepares and coordinates admission process for each individual and family admitted to Sun Behavioral. Demonstrates ability to maintain documentation as it relates to admissions and customer relations, by utilizing customer relationship software and EMR database software. Works with the clinical team, medical team, participant and family in developing a transition/discharge plan that is clinically appropriate and aligns with Sun Behavioral’s policies and procedures. Develops and maintains relationships with other providers and referral sources, as it relates to admissions duties.
